Abstract Background The clinical value of positive family history premature coronary artery disease (CAD) in risk prediction cardiovascular diseases is controversial. While an association with factors and manifestation has been described observational studies, it not implemented clinically established algorithms. Purpose We evaluated the CAD factors, presence obstructive CAD, long-term mortality. Methods present analysis based on ECAD registry patients undergoing invasive angiography at Department Cardiology Vascular Medicine University Clinic Essen between 2004 2019. For this analysis, we excluded all missing follow-up information. Self-reported was categorized as positive, negative, or unknown. Baseline characteristics were compared patient without history. Cox regression used to determine morality. Results Overall, data from 33,865 admissions (mean age: 65.0±13.1 years, 69% male) included. Positive 4,995 (14.8%) patients, negative 17,806 (52.6%), while unknown 11,064 (32.7%) patients. Patients significantly younger (63.6±12.4 vs. 65.9±13.3 p<0.0001), more frequently had diabetes (11.4 9.3%, active smokers (23.5 13.8%, p<0.0001).
